ORDER BELOW APPLICATION FOR REGISTRATION OF BIRTH OF
MALE:
This is an application filed by the applicant for
registration of his birth who was born on 01/06/1978 at HBT
Medical College and Dr. R.N. Cooper Municipal General Hospital,
Juhu, Mumbai, which is within the jurisdiction of this Court. The
Health Department of BMC, 'K-West' Ward have filed their say.
2 Perused the Affidavit alongwith photo copy of passport,
copy of school leaving certificate, translated copy of school leaving
certificate, affidavit of witness and copy of adhar card of witness.
All these documents produced on record show that, applicant was
born on 01/06/1978. There is nothing on record to disbelieve
contents the documents produced on record. Hence, I proceed to
pass following order: -

The concerned Sub-Registrar/Medical Officer of Health,
'K-West' Ward, BMC, Gr. Mumbai is hereby directed to register the
birth of the applicant namely “ Farukbhai Suleman Vali Delawala ”
who was born on 01/06/1978 at HBT Medical College and Dr.
R.N. Cooper Municipal General Hospital, Juhu, Mumbai, after
verifying the correctness of birth details. The applicant shall pay
the necessary fee including late fee.
