ORDER BELOW EXH.1
Applicant viz. Jerry Savio Dias has filed present application
under section 15 of the Registration of Birth and Deaths Act, 1969 r/w.
Rule 11 (1) of the Registration of Birth and Deaths Rules, 2000. According
to applicant he born on 23/02/1990 in Bombay Hospital, at Marine Lines,
Mumbai. However, in the birth register his name has been mentioned, as
'Jerry Dias' instead of 'Jerry Savio Dias'. It is further contended that his
mother's name has wrongly mentioned, as 'Mrs. Juliana John Dias'
instead of 'Juliana Filomena Raposo' and his father's name had been
mentioned as 'Mr. John Dias instead of 'Joao Januario Dias'.
2. The applicant wants to correct her name and his parents
name appeared in the register of birth maintained by the Medical Health
Officer H/East Ward, MCGM office, Santacruz (E), Mumbai.
3. The applicant in support of his claim relied documents i.e.
copies of his Birth Certificate and Baptism Certificate, his Passport, his
Mother’s and Father’s Birth Certificates, Baptism Certificates and
Marriage Certificate, his Aadhar Card etc.

5. Perused affidavit of applicant alongwith documents placed
on record. The applicant has requested to correct his name and name of
his parents in the register of Birth and Death maintained by MCGM. The

applicant in support of his contention relied upon the documents like
copies of his Aadhar Card, his Passport, Birth Certificates and Marriage
certificate of his parent etc. which shows that, the proposed correction in
the register is desirable. Therefore, I am of opinion that application is
deserved to be allowed. Hence, I proceed to pass following order.
ORDER
1. The application is allowed.
2. The concerned Sub-Registrar/Medical Officer
H/East Ward, MCGM officer, Santacruz (E),
Mumbai is hereby directed that, to correct the name
of applicant 'Jerry Savio Dias' instead of 'Jerry Dias'.
He shall correct the name of mother 'Juliana
Filomena Raposo' instead of 'Mrs. Juliana John
Dias' and name of father 'Joao Januario Dias' in
stead of 'Mr. John Dias' in the birth register, as per
provision of section 15 of the Registration of Birth
and Death Act, 1969.
