ORDER BELOW EXH.1
1. The present application is filed under Section 13 of Registration of
Births and Deaths Act, 1969.
2. It is the case of the applicant that she is married. Name of her
husband is Shrikrishna Jayasing Budhakar. She is residing at Room
No.306, 2nd Floor, D Wing, Janta Nagar C.H.S., Shivsena Shakha, Sion,
Mumbai. She delivered a female child, on 03.09.2005, who was named as
Ruchika Shrikrishna Budhkar, at L.T.M.G. Hospital, Sion, Mumbai. She is
having sufficient documentary evidence to prove her identity and residence.
3. The birth of child Ruchika Shrikrishna Budhkar, is not registered.
Her request to concerned authorities for registration of her name was
rejected. She is in need of such registration for availing various facilities
and services. Therefore, she prayed to order and direct the respondent to
register the birth of said child.
4. The application is supported by her and her husband's affidavit.
Applicant also filed on record copies of Ration card, Aadhar Cards,
Election Cards, of herself and her husband, papers regarding delivery of the
said child issued by L.T.M.G. Hospital, School and Leaving Certificate and
Higher Secondary Certificate Examination of the child.
5. From the contents of application and evidence produced it is clear
that the applicant delivered a female child, on 03.09.2005, at L.T.M.G.
Hospital, Sion, Mumbai, and she is in need of it for various purposes.
Nobody objected to the application and hence, following order -
ORDER
a. Application is allowed.
b. Respondent Ward Officer, G/North Ward, Dadar, Mumbai, is
herewith directed to register the birth of applicant's daughter named

Ruchika Shrikrishna Budhkar, as 03.09.2005, in record maintained by him
after payment of necessary fees by the applicant and issue birth certificate
accordingly.
c. Copy of this order shall be given free of cost to the applicant.
