ORDER BELOW APPLICATION FOR REGISTRATION OF DEATH.
1] The said application is filed by applicant Bal Krishna Naidu in
context of his mother Ullama Rammayya Naidu expired on 16/04/2019 at
Mumbai residence due to Termino Cardio Respiratory Failure. Due to
inadvertence the death of his mother could not get register to the department.
2] The respondent i.e. Medical Officer of Health, G/North Ward,
Dadar W, Mumbai has filed report and submitted that the death of deceased
Ullama Rammayya Naidu death on 16/04/2019 is not registered with their
office. The said officer further submitted that to scrutinize the case and
necessary order be passed.
3] Alongwith this application the applicant has filed on record his
affidavit stating every facts as per his application and also enclosed verified
copies of cause of death certificate, death report of deceased Ullama Rammayya
Naidu, Aadhaar card, ration card. Also, the applicant has filed on record
affidavits of Praful Shetva Kedar and Mangal Mohal Naidu who on oath stated

to have been present at the time of death of the Ullama Rammayya Naidu.
Verified Aadhaar Card copies of witnesses are filed on record as proving of
their identify and address.
4] On perusing the application, affidavit, documents of the applicant
in support of his application, affidavit of witnesses and report of BMC, this
Court does not find any hesitation to accept that 16/04/2019 is true and
correct date of death of Ullama Rammayya Naidu – mother of applicant. Hence,
proceed to pass following order.
ORDER
1 Application is allowed.
2 The concern authority 'The Registrar, Birth and Death, G/North
Ward, Dadar W, Mumbai400 028 is directed to take entry of the date of death
i.e. 16/04/2019 (Sixteen April Two Thousand Nineteen) of Late Smt. Ullama
Rammayya Naidu, in their register on accepting the fees as per the rules.
