an 	alternate 	address/physically 	verify 	the 	existing 	address 	and 	file 	an 	appropriate
affidavit 	in 	that 	regard. 	Heard 	and 	granted 	by 	way 	of 	last 	and 	final 	opportunity. 	Let
cost 	of 	Rs.1000/- 	be 	imposed 	on 	the 	complainant 	to 	be 	paid 	to 	the 	DLSA, 	PHC,
NDD. 	Receipt 	of 	the 	same 	be 	filed 	on 	the 	e-mail 	ID 	of 	the 	court.
Fresh 	BWs 	be 	issued 	subject 	to 	filing 	of 	the 	alternate 	addressin 	terms 	of
the 	last 	order 	upon 	filing 	of 	PF 	within 	15 	days.
In 	case 	steps 	are 	not 	taken 	matter 	shall 	be 	dismissed 	on 	NDOH.
